Tournament registration form

Having acquainted myself with SCBM tournament rules, I hereby agree, that by submitting this form, I understand the rules and will be bound by all decisions made by the tournament director at any said club tournaments,  I agree to assume all risks associated with my participation in above said tournament and release the SCBM  and any of it's sponsors, or officials from all claims to any damages incurred during any SCBM event.
